Transitions provides free advice and assistance to seniors and their families in need of finding the best senior living and care options. We offer friendly, local service, and arrange focused tours based on your care needs, budget, and personal preferences.
Transitions in Senior Living was founded in 2014 by senior advisor and referral specialist, Brian Sullivan. Brian works with his sister, Erin Moody, a 26-year veteran of the senior care industry, to provide the best information available about senior living and care options in Western Washington. Erin has over 20 years of experience in discharge planning and transitional care, helping families move their loved ones from nursing facilities into less intensive care accommodations. She also owns and runs two highly regarded adult family homes in Marysville.
When we began our company, we recognized an opportunity to improve upon the previously existing model of senior referral agencies and create a better experience for families and providers alike. We take pride in doing the job properly. We visit all homes in advance of touring with our clients; therefore, unlike some large referral agencies, we truly know the homes we recommend, and we know the care providers who run the homes. And, unlike some large referral agencies, we do not distribute your contact information to every provider in your area, so you won’t be bombarded with marketing calls. We personally offer our free services to you, and that’s it.
We enjoy helping families who are going through an inevitably tough time. The relief families experience when they find the right home for their loved one is extremely satisfying to witness.
Brian:
Founder of Transitions in Senior Living, senior advisor and referral specialist. Graduated with a BA in Psychology from the University of Washington in 1994 and received a Sports Management Certificate from the University of Washington in 2003. Worked as a professional golf caddie on the PGA Tour for 16 years (Across North America, Europe, South America, and Australia) and has done sports writing for the PGA Tour and the Seattle Times.
Erin:
Owner and operator of Holistic Adult Family Homes, 2008 to present. Graduated with a BS in Psychology from the University of Oregon, 1991. Geriatric Mental Health Counselor Snohomish Family Counseling, 1991-1993. Medical Social Worker/Discharge Planner in long-term care facilities, 1993-2008. Certified by the State to train caregivers in Dementia and Mental Health. Nursing assistant registered in the state of Washington. Qualified Assessor for AFH placement.
